Transforming a matrix into text with a certain format

Dear members,

I have 3 column matreces that I want to transform into into texts having the format shown in the picture below in an automated process. The 3 columns (containing the numbers) will be placed on the highlighted locations in yellow respectively. The other unhighlighted characters will remain the same. How can I possibly do this using MATLAB? (The text file is attached below)
