HDL code to simulink model

I have hdl code and how to make a simulink model out of it? the link to the hdl code is given below

You can use an HDL Cosimulation block to import this into Simulink. Here is a video and documentation for the procedure.

Could you please clarify whether you have a Simulink model or vhdl code? The link points to a Simulink model, but you say there is HDL code.
