Discrete event in Simulink

My system is this: I have a continous system running lets say a car following a path.
Whenever the car reaches the end of the path, i want a new path segment to be created and for the car to keep following that path instead.
How can i buil this discrete event of a new path creation, which is triggered only when the car reaches this certain state?
