Getting Values from c++ programm as simulink input

Hey, I got a c++ program which is calculating the position and i want to use simulink to control the position. Therefore I need to get the position values as input in simulink Now i have two options in mind:
1.I want to send the values from the c++ program to simulink directly. Is this possible?
2. Or write the data into a .m file and read it into simulink.
Do you guys have any idea if and how any of these options is possible?
