Paste the offset vector in your workspace. It reads like L_Link is already a 3 element vector and you are concatenating two 0's onto it. Try removing the 0's and running it.

In my experience that means your model is not setup appropriately. That one is a bit tougher to debug. Can you post a pic of your model?
