PID Simulink Block Parameters

I obtained the following control parameters using PID tuner app

Now I want to use these values for PID model in simulink, what would be the corresponding values of P, I, D and N using the above parameters?

Thank you for your response, I got the correct output when I used N=1/Tf, rest of the values were same as you metioned.
