Unable to perform assignment because the left and the right sides have a diffrent number of elements error for Simulink

what does this error mean when working with matlab Simulink. I searched for an answer but all questions were asked for a matlab code and unadequat assignement in particular instruction. and if in Simulink? any suggestions/help?
